    Documents Required

    • In India, doctors can be of three types: those who practice as a part of a hospital or nursing home, those who have an individual, professional practice by running their own clinic or by practicing in both a hospital and a clininc that is owned by them
    • For a doctor practicing as a part of a hospital or a nursing home, they would need to produce:
    • Aadhar Card, PAN Card or a Voter ID Card to establish proof of identity and proof of residence
    • A certificate from the IMA as well as the degree certificate from the institution from which the applicant obtained their degree in medicine
    • A statement of their salary from the hospital or a letter of employment from the hospital or nursing home that establishes their practice in the particular institution
    • For a doctor, who practices privately in their personal clinic, they need to produce:
    • Proofs of identity and residence in the form of Aadhar Card, PAN Card or Voter ID Card
    • A certificate from the IMA and the degree certificate from the medical college which conferred upon the applicant the qualifications necessary to be a doctor
    • A statement of income and expenditure made for the clinic in order to determine accurately the income of the applicant as well as legal documents that conclusively show the clinic to be a legal entity engaged in legal practice
